1. Form helps leaders of the team for staying in touch. Expect to be added to a low-volume email blast list. You can unsubscribe from this 412-list later, if you desire.
2. Agree to our model release terms as your photograph and videos can be leveraged in efforts with the programs, its sponsors, its partners and on Facebook and elsewhere. We take a bunch of photos. If you are in a witness protection program, this might be a bad choice of activities.
3. A backup contact is prudent too, just in case anything goes wrong. We want to know who to contact.
